NCLEX Application Form: New Jersey Board of Nursing

Please be aware that N.J.A.C. 13:37-2.3, the regulation pertaining to the application requirements for graduates of foreign nursing programs, was recently changed. The changes have been included in the application process.

There are five (5) elements that are now required for licensure of a foreign graduate as a nurse in New Jersey including:

A transcript review performed by the Commission on Graduates of Foreign Nursing Schools (C.G.F.N.S.) (You are required to have the Full Education Course by Course Report.);
Proof that the applicant has achieved a passing score on the Test of English as a Foreign Language exam, TOEFL (C.G.F.N.S. is also providing English language evaluations) (Must be submitted only to C.G.F.N.S.);
A completed licensure application for graduates of any foreign nursing program, which includes information concerning the applicant’s educational and experiential background;
Criminal History Background Check clearance; and
Passing the appropriate NCLEX licensing examination.

Please submit the following to the New Jersey Board of Nursing:

The official Application for Licensure by Examination;
One (2” x 2”) passport-type photograph;
The total fee of $225.00 made payable to the New Jersey Board of Nursing (a money order or personal check is acceptable); and
The Certification and Authorization form.

NCLEX Application Form New York State Board of Nursing

Applicants who have completed LPN or RN education outside of the US must have their education credentials verified by the Commission on Graduates of Foreign Nursing Schools (CGFNS). Such applicants should contact CGFNS at www.cgfns.org for the application and instructions to complete the CGFNS Credential Verification Service for New York State. Please note that this verification process is not the same as CGFNS’ Certification Program (which is required for registered nurse applicants applying for limited permits). The CGFNS Credentials Verification Service for New York State licensure applicants is a process for verifying the authenticity of education credentials. The CGFNS Credentials Verification Service for New York State does not indicate approval by the New York State Education Department of the content of education. After education credentials are verified by CGFNS, they are then forwarded to and evaluated by the New York State Education Department.

You will be advised in writing of the results of the New York State Education Department’s evaluation of your education. In addition, the licensing examination administrator will be notified if you are eligible for the examination.

Steps for International RN Applicants

Here are the basic steps to become a California RN if you were educated in another country, and have never been licensed in another state in the U.S. (If you are licensed in another U.S. State and have passed the U.S. licensing exam, you may qualify for endorsement licensure.)

Read detailed application instructions and download forms at this link. Or you may request an application packet by contacting the Board of Registered Nursing.
Submit your application form, with appropriate fees. You will need to provide a U.S. Social Security number before a California license can be issued to you.
Submit your fingerprints for a criminal background check.
Submit a copy of your license or diploma that permits you to practice as an RN in your country.
Have your college transcripts sent directly to the California Board of Registered Nursing (BRN). If your transcripts are not in English, there are special instructions for translation.(pdf)
If your college has never submitted a transcript to the BRN for evaluation, the BRN will also need the college’s curriculum, catalogs, or other documents for evaluation.
Apply for an Interim Permit if you wish to work under supervision temporarily while your licensing application is being evaluated. If you are from a non-English speaking country or did not take your country’s licensing examination in English, you will need to take an English comprehension examination to qualify for an Interim Permit. Take and pass the National Council Licensing Examination (NCLEX). The exam is computerized and given continuously 6 days a week.

NCLEX Application Form Vermont Board of Nursing

Here’s a quick step by step process on how to apply for licensure by examination (NCLEX) at Vermont Board of Nursing for graduates of international nursing schools, including the Philippines.

1. Visit www.vtprofessionals.org and download the 13-page International Nurse Application for Registered Nurses. The first 4 pages are the instructions on how to fill up the form.

2. Fill up pages 5-10 (page numbered 1-6) to the best of your knowledge. Paste your 2×2 picture on the space provided in page 5. Page 10 is the Secondary Education page, where you will attach your original or certified true copy of high school diploma/certificate. You will also include in this packet your photocopy of license (front and back in one page) certified by PRC, photocopy of your passport (face page), $150 Application fee in bank draft, money order, etc.

3. Fill up the upper part of page 11 and 12 (Verification of Education) and sign on the space provided. Bring these pages to your school of nursing and have them fill it up, stamp and enclose these in school envelope, with the transcript of records, RLE summary and detailed course descriptions. Then they usually give it back to you for mailing; you can mail it together with your other forms as long as it’s enclosed and sealed in an envelope provided by your school.

4. The last page is your Verification of Licensure. Fill up the upper part and sign. Bring it to PRC for completion. The PRC will mail it directly to Vermont.

Processing and evaluation of your application ranges from 2 weeks to 4 months depending on a lot of factors. If you’re approved and eligible, they will send you an email about the next step, which is NCLEX registration at Pearsonvue.com.

NCLEX Application Form Florida Board of Nursing

If you were educated in a foreign country and are applying for exam, you are required to have your nursing education evaluated by one of the approved agencies listed below. Contact the agency of your choice and request the Full Education Course by Course Report. An original copy of the report must be sent to the Board of Nursing directly from the agency. You are responsible for all fees the agency charges for these services. After your application for licensure is processed and has been deemed complete, the Board of Nursing will review your educational evaluation and contact you with the status of your application in writing. Please ensure that your mailing address is up to date throughout the application process.
